Home Health Authorizations Manager: Verifies insurance eligibility and authorization of current and potential home care clients in an accurate and timely manner. Communicates payer authorization and eligibility information to Clinical Manager, Business Manager or designee.
Home Health Intake Coordinator: Serves as the first line of customer service for internal and external customers. Receives and documents patient referrals to home health/hospice/private caregiver services, ensuring compliance with applicable coverage criteria. Reviews referral for appropriate care setting and communicates with care center, business development and referral source when appropriate. Gathers appropriate information to assist the clinical team in scheduling the evaluation visit. Communicates pertinent patient information to the care center team via approved processes. Coordinates with other departments for insurance verification and payer authorization as needed. Professionally communicates and follows up with referral sources regarding required documentation for a complete referral. Meets productivity expectations with quality documentation of complete referrals including all required follow-up. Coordinates the evaluation/admission process. Performs other duties as assigned.
Clinic Setting Scheduler: Scheduling appointments Using a computerized or manual system, clinic schedulers schedule patient appointments that align with the clinic’s policies and the patient’s preferences. Collecting patient information, clinic schedulers obtain and update patient demographics, insurance information, and other relevant details. Communicating with staff, clinic schedulers communicate with physicians and other staff about patient concerns or scheduling issues. They also coordinate with healthcare professionals about schedules and changes. Managing patient accounts, clinic schedulers maintain patient accounts by recording and updating personal and financial information. They also collect patient charges and control credit extended to patients. Providing customer service, clinic schedulers provide courteous and professional service to patients and providers. They may also answer patient questions and anticipate their anxieties. Performing other duties, linic schedulers may also perform reception duties, such as answering phones, screening calls, and taking messages.
Home Health Case Manager (support): Registered nurse (RN) licence or other relevant healthcare certification depending on the role and agency. Strong clinical assessment and care planning skills. Excellent communication and interpersonal skills to interact with patients, families, and healthcare team members. Case management experience, particularly in home health settings. Ability to work independently and manage a caseload effectively. Knowledge of community resources and referral systems. Proficient in electronic health records and documentation.
